The purpose of the interactive NHVRIN web query system is to provide access to vital statistics databases and support the information needs of data users such as local health agencies, the New Hampshire Department of Health and Human Services, hospitals, legislative researchers, educators and students in improving service delivery, evaluating health care systems, and monitoring the Vital Records of the people of New Hampshire. These databases may be used only for the purpose for which they are provided. Any effort to determine the identity of any individual, or to use the information for any purpose other than for statistical reporting and analysis, is prohibited. Therefore, as a condition for being given access to data provided by the Division of Vital Records Administration (DVRA) over the internet, I agree that:
  • Use of these data shall be for statistical reporting and analysis only.
  • No attempt shall be made to learn the identity of any individual included in these data.
  • No disclosure shall be made or other use of the identity of any person discovered inadvertently.
  • If an identity of a person is discovered the Director of the DVRA MUST BE NOTIFIED IMMEDIATELY.
  • While accessing the databases, my activity will be monitored and recorded for auditing purposes.
I understand that failure to adhere to the above stated agreement items will result in loss of access to the DVRA internet databases, and I may be subject to legal penalties. Any fraudulent use, release, or publication of vital statistics data contrary to the provisions stated is a class B felony, with penalties that may include a criminal fine of up to $4,000 and a maximum sentence of imprisonment of 7 years. If I am a New Hampshire state government employee, this may be grounds for immediate dismissal.

Press the [OK] button if you accept the stated conditions or press the [Cancel] button to decline.
